Health Services

All kindergarten students entering District 202 are required to have a kindergarten physical exam on an Illinois Child Health Exam form, dated after August 24, 2008, listing all dates of required immunizations.  This form must be brought to your child’s school before classes begin on August 25, 2009. This is an Illinois Department of Public Health and District 202 requirement.  Children with missing health requirements will not be allowed to start school on August 25, 2009. 

All children in Kindergarten must have an eye exam completed within one year prior to starting school in the fall by a licensed optometrist or medical doctor who performs eye exams as specified in the Illinois Department of Public Health administrative rules. This form must be submitted by Oct. 15, 2009.

Additionally all children in Kindergarten will be required to have a dental health exam performed by a licensed dentist.  This form is to be submitted to your child’s nurse before May 15, 2010. 

Local health, dental and optometric resources who offer services based on a sliding scale and/or accept Medicaid and KidCare are available from your school’s nurse.
