District Health Forms

Letter to Parents about Head Lice


All kindergarten students entering District 202 are required to have a kindergarten physical exam on an Illinois Child Health Exam (English | Spanish) form, dated after August 18, 2011, listing all dates of required immunizationsThis form must be brought to your child’s school before classes begin on August 16, 2012. This is an Illinois Department of Public Health and District 202 requirement.  Children with missing health requirements will not be allowed to start school on August 16, 2012

All children in Kindergarten must have an eye exam completed within one year prior to starting school in the fall by a licensed optometrist or medical doctor who performs eye exams as specified in the Illinois Department of Public Health administrative rules. This form must be submitted by Oct. 15, 2012.

Additionally all children in Kindergarten will be required to have a dental health exam (English | Spanish) performed by a licensed dentist.  This form is to be submitted to your child's nurse by May 15, 2013

Local health, dental and optometric resources who offer services based on a sliding scale and/or accept Medicaid and KidCare are available from your school’s nurse.

All incoming 6th and 9th grade students in District 202 are required to have a physical exam on an Illinois Child Health Exam (English | Spanish) form, dated after August 18, 2011, listing all dates of required immunizationsThis form should be brought to your child’s school before classes begin on August 16, 2012. This is an Illinois Department of Public Health and District 202 requirement.  Children with missing health requirements will be excluded from class on the first day of school.  It is highly recommended that you return school physicals to school by August 1, 2012.

All students in 6th grade must have a dental health exam (English | Spanish) performed by a licensed dentist.  This form is to be submitted to your child's nurse by May 15, 2013. 

Local health and dental resources who offer services based on a sliding scale and/or accept Medicaid and KidCare are available from your school’s nurse.

Physical Examinations

Illinois State Law and District 202 requires students entering Preschool, Kindergarten, 6th grade, 9th grade, and/or when transferring from an out-of-state school to have their required physical exams and immunizations completed by the first day of school, or they will not be allowed to enter class. Appointment dates will not be accepted. The health history portion of the physical examination form must be completed and signed by the parent in order for the physical to be valid.
