Health Services

All incoming 6th and 9th grade students in District 202 are required to have a physical exam on an Illinois Child Health Exam form, dated after August 24, 2008, listing all dates of required immunizations.  This form should be brought to your child’s school before classes begin on August 25, 2009. This is an Illinois Department of Public Health and District 202 requirement.  Children with missing health requirements will be excluded from class on the first day of school.  It is highly recommended that you return school physicals to school by August 4, 2009.

All students in 6th grade must have a dental health examination performed by a licensed dentist.  This form is to be submitted to your child’s nurse before May 15, 2010. 

Local health and dental resources who offer services based on a sliding scale and/or accept Medicaid and KidCare are available from your school’s nurse.
