Forms & Handouts

Class handouts and other forms from class will be made available on this page. All forms are available in class or from a band director. While we will attempt to keep links current, handouts given in class should be the primary source of information for parents and students. Some information will be given in class that is not available on this website.

In general, all forms should be submitted to the "blue box" located in the PSHS Band Room.

General Forms:

  • Alternate Transportation Form: This form must be filled out if a student wishes to ride home with a parent from any band event.
  • Challenge Form: Students who wish to challenge for a higher chair may fill out this form and turn in to a band director.
  • Conflict Identification Form: To report a conflict with a scheduled band event, please fill out this form and submit to a director.
  • Excused Absence Request Form: To request an excused absence, please fill out and submit this form. A director will then review the form and determine if the absence will be excused.
  • Instrument Check-out Form: To check out an instrument owned by PSHS, you must complete this form and submit to a director for approval.
  • Student Account Expenditure Form: Monies from your student account can be applied towards purchases (such as marching band shoes, uniform cleaning fees, reeds, etc.) by completing this form.
  • Trip Consent Form: This form must be filled out for each trip away from PSHS. Please check with a director for due dates. In most cases the day of the performance is too late!
