Transfer Students

Physical Examinations

Illinois State Law and District 202 (see District 202 Health Policy) requires students entering Preschool, Kindergarten, 6th grade, 9th grade, and/or when transferring from an out-of-state school to have their required physical exams completed by the first day of school, or they will not be allowed to enter class. Appointment dates will not be accepted. The health history portion of the physical examination form must be completed and signed by the parent in order for the physical to be valid. Transfer students have 30 days to turn in their immunization record, provided they submit proof of appointment for their immunizations.

Local health and dental resources who offer services based on a sliding scale and/or accept Medicaid and KidCare are available from your school’s nurse. 
 

Eye Exams

All students enrolling for the first time in District 202 must have an eye examination completed within one year prior to starting school in the fall by a licensed optometrist or medical doctor who performs eye exams as specified in the Illinois Department of Public Health administrative rules. This form must be submitted by Oct. 15.
